The keyword here is annotated. Yes, I would provide the text with changes tracked, however, somewhere on the sheet, you'll also want to explain how, why, or what was changed (as appropriate). Otherwise, what you have is simply a change page.
In my experience, errata sheets can take different forms depending on the layout of the original text, the nature of revisions requested, etc. It all depends, so I think it is important to ask clarification questions, if you have the option to do so.
Sometimes it is appropriate to simply refer to the goverment's question/comment (that precipitated the revision) at the top of the page and then provide the revised text below that. At other times, you might need to use a two- or three-column table to document your revisions.

Thanks. I think I am going to scan the handwritten notes I might be able to reuse, then per your suggestion, move everything else to a CD. It's just hard for me to throw out what took a lot of effort to create and maintain. But then again, I don't want to become the next subject on Hoarders. ;-)

In the past, we have used SharePoint's Survey feature, which allows individuals to anonymously answer questions online. SharePoint allows the administrator to view individual responses, as well as overall responses (in a graphical display). The only problem has been having to hound people to actually take the survey.
We mainly use multiple-choice questions requiring a response of Strongly Agree, Agree, Disagree, Strongly Disagree, or Don't Know. We also try to include one open question requiring a free-text response.

We don't have a lot of spare space at my office, and I am inundated with paper. I am looking for opinions on what you typically save from your proposal planning binders, if anything. How long do you keep your binders around? I've not thrown anything out for 2 years. Thanks!
